Hazard, who has been strongly linked with a move to Real Madrid this summer, evaded five players before Belgian forward calmly beat keeper Lukasz Fabianski.
The strike took his tally to 16 in the EPL this season equaling his best ever finish in the league during the 2016/2017 season.
Chelsea, who can still qualify for next season’s Champions League by winning the Europa League, are three points ahead of fifth-placed Arsenal, who have a game in hand, after a third successive league win.
West Ham remains 11th after a third defeat in four league games.
The Blues face Slavia Prague in the Europa League quarter-final first leg on Thursday while West Ham are away at Manchester United in the Premier League on Saturday.
